EMERGENCY PLAN The purpose of this template is to capture all the information necessary to provide for a child/young person should a situation arise whereby their main carer is unexpectedly not available e.g. taken ill, admitted to hospital etc. It is important to include sufficient detail to cover the variations that may occur on different days of the week e.g. weekdays vs weekends.
Tread carefully. Don’t forget you are asking parents, usually a mother, to face her worse nightmare i.e. what would happen to their son or daughter if they (mum) were not there e.g. rushed into hospital or worse?
This document can never be finished. As more things are learned about the person and how they communicate and behave as they grow and as their life changes, it should be updated to make sure it always reflects and accommodates all of this so that it remains relevant and current.
